Volunteer Waiver
This Waiver MUST Be Completed for You to Volunteer
Your participation as a volunteer is based on your acknowledgement of and agreement to the following conditions: Read this form carefully before agreeing. Volunteer work for Paddlefest is a potentially hazardous activity. A volunteer should not participate unless medically able to participate. By agreeing as indicated below, you agreet to all of the following covenants and conditions. Each volunteer assumes all risks associated with volunteering for this event, including but not limited to the effects of weather, traffic, venue conditions and surfaces, falls, and contact with other volunteers, paddlers and spectators. I am aware that medical support for this event will be provided by volunteer personnel who may be called upon to provide assistance, including first aid, to me as a volunteer during or after the event. I authorize any such volunteer medical support personnel to assist me or to perform such assistance as in the opinion of such person may be necessary or appropriate.
I for myself, my heirs, successors as assigned and any person entitled to act on my behalf, do hereby release The Ohio River Way, Inc., Paddlefest, Coney Island, City of Cincinnati,United States Coast Guard, Cincinnati Parks Department, all sponsors, volunteers, charities, Paddlefest event staff, directors and officers, together with their subsidiaries, successors, heirs, contractors, subcontractors, directors, officers, agents, attorneys and representatives from all claims or liabilities of any kind or character whatsoever arising from my participation in Paddlefest Weekend, June 23-25, 2011, or any of its allied or accompanying events or in any related activity. I consent to the use of my image in photos, video and audio recording, film, of my participation in Paddlefest Weekend events.
Registration for minors must be completed by a Parent or Legal Guardian, who agrees to all of the foregoing on behalf of such minor.
If your age on the day of the event is less than 18 years old, please fill in the Parent/Guardian Contact Information below. Providing such information and continuing this process indicates that this volunteer registration was completed by the person listed below.
